Our Athletic department is committed to building strong character, promoting sportsmanship, and developing student-athletes both on and off the field. Eligibility & Physicals

All Athletic programs will stress academic success, physical fitness, character development, discipline, and good sportsmanship. Our athletes represent our school and community and are expected to be student leaders with positive behavior. Students who use alcohol, tobacco, or other drugs, have behavior or attendance problems, or violate other training rules as set by their coach, may lose their opportunity to participate.

Eligibility 

Students must maintain a 2.0 GPA or higher to be eligible to participate in athletic programs. For athletes to participate in daily practices and/or games, they must be present for at least four consecutive periods on that day. Students must also complete a satisfactory medical examination during each academic school year in which they plan to participate.

 

Physical Information

All potential student-athletes must have a completed & cleared sports physical to participate in tryouts for their chosen sports. Just uploading the physical does not mean a student is able to tryout or participate.

1. FHSAA EL2 Preparticipation Physical :

Required annually, valid for 365 days

Online Physical Submission Instructions via AKTIVATE:

1. Click on the link under documents for step-by-step instructions on how to create an account.

2. Go to the link (sign up Today for Sports) under documents.

First-Time Users: Create an account with a valid parent/guardian email and password (parent/guardian email MUST be used).
​Returning Users: Enter login information. Use the same account to retain prior information.

3. Upload Required Forms:

Physical on FHSAA EL2 (NEW FORM REVISED 2/25): Returning Users: Enter login information. Upload Page 4 (and Page 5 if Referred). Ensure completeness: all sections, parent/student signatures, doctor's date/signature, and doctor's office stamp with address.

  • When registering your athlete, make sure to choose all the sports for which the athlete is planning to try out.
  • Sports physical are good for multiple sports 
  • ECG form is not required for middle school.
